The Supplemental Questionnaire (SQ) will be used to pre-screen applicants for OYCRs Health Policy Division vacant HPS II position. The SQ must be no longer than three (3) pages in length, using 12-pt. Arial Font. Applications submitted without the SQ will not be considered for an interview.
• Provide examples in narrative form of your experience planning, coordinating, and delivering training and technical assistance.
• Provide examples in narrative form of your experience leading a team or workgroup to produce a product e.g., grant application, educational material, policy campaign, and training.
• Describe your knowledge and expertise associated with interpretation and application of health-related laws, regulations, and programmatic oversight of a government and/or non-government program.
For experience/education to qualify during the application screening process, and to ensure that minimum qualifications can be determined, applicants should include all employment history on the Employment Application (STD 678) and/or Resume, including detailed job descriptions, hours worked per week, and start/end dates (MM/DD/YYYY). Training and Development assignments are considered limited term, 12 months, and may be extended up to 24 months or become permanent.
Candidates who reside outside of the State of California may be admitted to the job interview, but they must provide proof of residence in California prior to being eligible for appointment.
